Shawano Pharmacy is owned by the 8 First Nations that comprise the Southeast Tribal Council also known as Southeast Resource Development Council (SERDC). Each Nation is an equal shareholder in Shawano Pharmacy but also receives proportional profit sharing from the actual transactions they direct to the pharmacy.
The first Consent For Transfer Form is for adults, the second form is an Under 18 Form for minors and children in CFS care. Please select the appropriate form - only one needs to be submitted. Authorized Consent: I authorize SHAWANO PHARMACY to obtain all prescription profile and information relevant to my pharmacy needs.
