We provide community-based mental health services for adults with serious and persistent mental illness. All services support and empower those with mental health challenges to recover and live well in the community. Our services are available through referral to Intensive Case Management Access Coordination (IntAc). Hamilton Mental Health Outreach is a registered charity and welcomes donations for special projects and client support.
Services
Hamilton Mental Health Outreach has provided a supportive and nurturing environment for persons with challenging mental illness since 1988. Our organization was created to provide support for individuals discharged from psychiatric hospital beds to the community. For most of its history, the agency was known as Wellington Psychiatric Outreach Program and was located on Wellington and King William Streets in Hamilton.
Hamilton Mental Health Outreach is a non-profit mental healthcare provider, meaning all services are provided free of charge. Our team of intensive case managers provide and coordinate various services to help clients along their journey of recovery. Intensive case management services are personalized to the client's recovery plan.
IntAC is a centralized process and single point of access to mental health intensive case management services for residents of Hamilton to persons, aged 16 years or older, who are living with a serious and persistent mental illness. We are among 4 agencies who receive referrals thought this process.
