As a collaborative team, we strive to be an innovative, integrated team providing comprehensive patient-centered care to a richly diverse and varied ethnic population. We will strive for excellence in chronic disease management, clinical care, patient and medical education and use of the latest medical technology.
A group practice means you will have one primary doctor who will provide continuity of your care, but we work in a health care team model. Our other doctors and health care team members can serve as a temporary support until you see your own doctor. We specialize in advanced access care, planned proactive chronic disease care, and clinical pharmacist reviews.
You just need your personal information such as your full name, care card number and birth date to make an appointment! You can book 1 appointment, ONLY with your own health care provider. You should NOT BE COMING TO THE OFFICE as these visits are Telemedicine visits. Telemedicine Visits are remote visits, which means that you can see your Personal Health Care provider without leaving your home.
